This is an outstanding booklet which most of my planning is based around for years 3 - 6. Students enjoy using it and finding out where they are, where they want to be and how to get there.
It has peer, self and teacher assessment which fits in with the new English curriculum.

Students tick the boxes down the left hand column for what they can do.
Whatever they don't tick is their target for next year.
Instructions for the Evaluation sheet
If one or two boxes are ticked then it is in the almost
If three or four boxes are ticked it is in the Got it!
If five or six boxes are ticked then it is in the Wow box
Use their year group instead of a tick on the evaluation page and then you can see their progress from year 3-6.
