Reading Record Sheets - For classroom use

I designed these reading record sheets for classroom use by any adults that listens to my pupils read on a 1:1 basis in school. I printed out an assessment sheet for each pupil at the sublevel they were working towards and asked that the adults ask questions based on the 'I can&' assessment focus statements. This evidence is then documented in the table below. I&';ve found that this document is useful evidence for reading APP.
