SEMH individual progress tracker

A useful tool to aid teachers and support staff who are delivering SEMH intervention to monitor and track sessional progress.

This spreadsheet allows you to enter a session-by-session RAG record of how students have accessed the intervention, with options to add further steps/ steps for future learnings.

You can then use this record to create a complete overview of their time on the programme, which can then be used as supporting evidence to signpost onto additional interventions if necessary.

School leaders may also use this as a prompt to use for meetings with parents or external services so underpin specific areas where specialist or targeted support is required.
