This FREE resource allows for all of a students key information to be stored on one piece of paper so that all staff can be in the know of how best to support a young person achieve their goals. This resource is student lead, providing empowerment and promoting independence to students who can sometimes go without this. It’s an opportunity to note any key struggles, sensory issues, triggers and motivators a student has and how best to maximise engagement in the classroom.
If a student isn’t able to fill out the resource themselves then having the support of a familiar teacher/friend or parent could be the way forward.
For students with SEN consistency is key. This fact file can be uploaded onto your school or colleges shared data base so that all staff are in the Know about how best to support individuals. The student support fact file can also be photocopied once completed and handed out to key members of staff (LSA’S, SENCO, HOY etc.)
