INTERVENTION LOG - PLANNER

Interventions especially in year 1 can be a nightmare with phase 2-3-4 and 5 for different groups of children and individuals.

Therefore, I have created a planner to help keep you organised!

Also a great way to show teachers, phonic leads and SLT that you are tracking the sessions and anyone can carry on where you left off if you keep record.

There are 37 pages BUT if you have more than one group for one particular phase, you can print as many sheets as you need - you will have the PDF.

It comes with group pages for phase 2-3-4-5 for each term and a phase 5 individual focus sheet.
Tick the sessions you complete as you go along. eg - Phase 2 additional blending is 7 weeks and phase 3 is 5 weeks having a circle for each day of the week to tick off.
Also, space for the children’s name in particular groups upto 8 names.
