Team work self assessment tool (Dance, Drama, Music, P.E. Food)

For students to complete individually as a way of self assessing their team work abilities.
This resource includes:
-a 16 item tick box reflection (likert scale from strongly agree to strongly disagree)
-a chance for your students to tell you who they prefer to work with when working in groups.

The biggest factor that determines success in Dance, Drama, Music is a students ability to work with others. I found over the years, breaking down these skills has been really useful so students understand what it takes to be a good team player.
This has also saved many an argument over who to work with (especially in Year 7)
I use it at the beginning of each term so students can track their progress.
I also chose a team work skill as a focus for each lesson, demo it, say where I see it and praise accordingly.
Hope it’s useful :-)

Students are tested on 22 key words. Really useful for Year 9 and up, but can be edited very easily. I use it as a starter at the beginning of the lesson or as homework. It can also be used for cover work.
I hope you find it useful.
