This resource is great for teenagers and young people who want to understand and address changes. The Stages of Change model is frequently used in drug and alcohol work which can be helpful to support teenagers and young people who want to understand and address changes. With that said, any type of change that they would like to be addressed can be explored using this template.

This Activity introduces the stages of change model with clear explanations of the various stages. The activity includes providing a step by step template which scaffolds the individual through the various stages for a particular change that they would like to address.

Reflective questions and rating scales are provided and support the individual to gain insight and judgment as to their stage of change. This resource includes the following:
