Patient Record Form Activity

Activity I do with pupils to illustrate the importance of forms to help gain information about a patient. I split the class into groups, with one 'patient' per group. The patient is given the patient information card and told that they must not give any information out unless asked for it. I give half the groups the record card and half a blank piece of paper (but sit them apart so they don't realise). They have two minutes to write down information they think a Doctors' Surgery would require from a new patient.
