These very basic worksheets give students the chance to familiarise themselves with the Proof of Age card. They have spaces to add a photo and write or stick on their name and date of birth. On the ones I am using with my students I have added our school logo and name in the bottom footer to replicate the extra information at the bottom of the actual PASS cards.
