Here is a SEND Pupil Passport that I have created for use in my school. This is used effectively and placed in the individual records of pupil’s, so all staff including those from outside agencies get an idea of the pupil’s needs. These are also provided to class teachers in July ready for transition, so they get an overall idea of the child’s needs and how best to meet them.
