I have created this sheet to go inside a plastic folder. Every teacher who has an additional adult will complete this so when the additional adult arrives in lesson, they can see some basic information about the lesson. The additional adult completes the bottom section within the lesson and so after the lesson the teacher can see anything that was not able to be communicated during the lesson. This can also be used as evidence for the amount of support some students may need. This has worked really well in a secondary school where there are lots of teaching staff and additional adults. A form that I have created to send out to teachers of a student who has been referred to the SEN department. This is built on the main areas of need and will give you an idea of concerns about a particular student on a whole school level. It is quick for teachers to fill out as it is mainly tick boxes and comment columns but it means colleagues are commenting on the particular area you need them to.

Literacy cube that I have created for students to have in front of them instead of a literacy mat. There is a copy of our marking policy on one square but you can easily delete this and add your own. Students love these as there is just enough information and they can turn the cube to the section they need. It has really helped them develop more independence. Information on faces are: - Marking policy - 3 things to do before you tell me you are finished - 3 things to do before you ask me for help - Key punctuation - Parts of speech (adjectives, verbs etc) - TiP ToP (paragraphs) All faces are easily adaptable. Cubes need printing, cutting and gluing. Card holds well. I have mainly used with bottom set year 7 but could work higher up.

Detailed information sheets for special educational needs including Dyslexia, Dyspraxia, Dyscalculia, ADHD, ODD, Asperger's, Autism, HI/VI; Attachment Disorder. All sheets include a brief description of the need, traits students may show and top tips/ strategies to use in the classroom. I have also added links to useful websites at the bottom. These can easily be printed to put in planners/ folders or blown up for staffroom displays
