Job Summary
We are developing a role that will facilitate the effective transition for students joining Horizons Academy, and being a key individual when a child reintegrates back into mainstream education. Funded by the London Mayor’s Violent Reduction Unit (VRU), this is a new role that takes responsibility to support students as they undergo this significant transition and provide holistic support to the student and their families.
This role will require the successful applicant to work closely with a range of colleagues within the Outreach Team, with the Head of Outreach, the VRU Project Lead, SENDCO and Head of Student Services to ensure a smooth induction process into Horizons Academy, to develop an understanding of their academic, social, emotional and mental health needs. The Transition Co-ordinator will also be responsible for updating and monitoring students, part of the team around the child that help to set realistic SEMH and academic targets so students are able to demonstrate progress. The post-holder will also be expected to work alongside the Safeguarding team in order to protect and uphold the welfare of new students.
Based in the London borough of Bexley, our academy specialises in alternative provision supporting up to 117 pupils for whom traditional teaching methods have not worked. Students join us following permanent exclusion from a mainstream school or due to medical needs, or social, emotional and mental health needs, that prevent them from attending a mainstream setting. Our mission is to provide our pupils with creative, nurturing and inspiring opportunities which reengage them in learning. By providing a safe and engaging environment, we endeavour to remove barriers and promote the opportunity for students to excel and reach their full potential. We aim to provide a broad and ambitious curriculum, catered to the needs of each individual, delivering the knowledge and skills that our pupils need to succeed in education and beyond.

About Horizons Academy Bexley
Horizons provides high quality support to pupils in Key Stages 1–4 who have been permanently excluded (or are at risk of exclusion) from mainstream school.
We focus on early intervention, offering a range of services from short term respite to longer term education for students in KS4. Our expert team works intensively with post-16 pupils to secure destinations whilst providing ongoing support. This helps ensure that all pupils can maintain their educational placements, training programmes and/or employment.
Our aim in Key Stages 1–3 is to re-integrate pupils back into a mainstream or specialist school via our ‘Refocus programme’. We work closely with external agencies and our partner schools to identify the needs of the child and develop the child’s own ability to learn, so that they feel confident about being in the classroom.
We have created an environment and developed a curriculum which supports our pupils to become well rounded citizens and able to productively contribute to the community. We want all our young people to feel confident about their future and in their own ability to live a successful, meaningful life.
