Phonics Screening Assessment Score Grid

This an editable resource that you can use to input the names of the children in your class and keep track of their phonics scores throughout the year. I absolutely love a colour code! It not only brightens up my screen, it also allows me to see at a quick glance who is off track and who I need to target with intervention. There is no better feeling that seeing those reds in September turn to green by the time for the screening!
I hope this document helps to make your life a little easier. If it does, please pop a comment in the review section. I really value your feedback.

This editable document is perfect for helping children who are easily distracted get back on track with their learning. It can act as a reminder on their desk, a teacher prompt to check in on key children or display in the classroom.
I have left some areas incomplete so you can tailor the resource to best fit the needs of the children in your class. I found it has worked successfully for children who need support with concentration or are reluctant to work independently. It helps to set clear boundaries and gives the children using it multiple opportunities to get back on track.
If you find it helpful, please drop me a review. It would be really appreciated!
