A passport that I have found particularly useful when starting practical lessons within D&T. This one is specifically based around resistant materials but it could easily be adapted to suit other areas.
The idea behind it is for every pupil to be given one at the start of the module or year and when pupils have been shown fully how to use the tool or equipment by yourself in a demonstration they then fill out the risk assessment that is printed on the reverse. It is then signed and dated to prove that each pupil has had sufficient training on the equipment or tools.
