EHCP application checklist and evidence gathering

A handy, easy to use checklist for when applying for an EHCP needs assessment. The checklist allows you to map out what needs to be done as accompanying guidance explains the importance of each document and what will make it effective. I have also attached an example of a chronology of interventions and advice as well as an example provision map and provision proposal.
