SEND Individual Provision Map

An Individual Pupil Provision Map to monitor interventions for SEND pupils. Interventions and assessments can be edited to personalise for individual school needs. The key feature of this IPM is that it is an ‘at a glance’ snapshot of the provision that a pupil is receiving for each term of an academic year. It is a very useful monitoring tool and can form part of a SEND child’s paperwork to share with parents, carers and external professionals.

In our school, this provision map works alongside a Support Plan and a One Page Profile (see other listings) to form a child’s One Plan paperwork.
