Hallmoor School caters for the needs of an increasingly diverse pupil population with a wide range of educational difficulties, ranging from moderate, severe, and complex needs. Some pupils also present with communication, physical, sensory, behavioural, or autistic difficulties requiring additional adult support and specialist intervention.

We are looking for a passionate and self-motivated Education, Health and Care Plan (EHCP) Coordinator on a full-time basis (36.5 hours) to work with our students and staff at Hallmoor School.

As a EHCP Coordinator you will be responsible for the coordination of the EHCP review process. This will be for our existing learners and those due to join the school at the start of the next academic year. You will also be required to contribute to the co-ordination of exams, data and assessment related to the young person short term and long-term targets.

 

You will work closely with a range of stakeholders to ensure EHCPs are of good quality. You will be responsible for them being up to date and completed in a timely manner in line with the SEND Code of Practice. As well as coordinating EHCPs the role will take on responsibility for bringing in referrals to the programme. You will confirm all learners accepted on to the programme are suitable, to ensure we are supporting learners to achieve paid work outcomes by the end of the academic year.
