A simple-to-use Individual Education Plan which will help teachers assess, plan, do and review their provision in place for any child who requires additional support or who is on the Special Education Needs register. It can be used for a short period as a tracker for interventions, or can be used for teachers, parents and children to work towards the outcomes on their EHCP. This is an example of initial T1-2 targets and how I would expect it to be completed.
