I created these sheets for gathering evidence for reading teacher assessment. They are based on Year 5 and 6 objectives for reading. I have used them in both guided reading and independent reading time.

The children can use these to record their independent reading and this also provides evidence for some of the reading objectives for Y5/6. There is an example sheet included too.
