SEN Case Study sheet

(8)
This is a sheet I've formulated using a variety of resources. It can be used as the start of a Case Study of a child for Ofsted and also used to track each child on the SEN register. I&'ve added a slightly different sheet (1) and an extra sheet. Ofsted liked these. :) Also, an Educational Psychologist has seen them and recommended they are submitted with an Appendix B to show intervention, discussion etc.

I use these little grids at the end of a lesson. The children look at the success criteria for the lesson and then mark their own work and give themselves a next step. They also circle the thumb that is relevant to their learning. The peer assessment sheet is very similar but it allows them to write something they liked about their friends work and a place to write a next step for them. The end of unit assessment sheet is useful if you wish to find out what you might need to go back to. Prep by typing in relevant information then copying and pasting it a further five times on the page.

Day Plan Templates

(1)
I have used these for quite a while now and they allow me to include quite a lot of detail for a day plan without having to have my weekly plans out. I can also see and show cross curricular links easily, and write down any notes or observations. At the bottom of the grids, there is space to put in a carousel of guided reading activities, early morning work and thinking time work. These can be easily adapted to your individual needs. The day plan with homework has an extra box to place any homework activities in.

My take on the new Code of Practice. I am trialing this format in my school at the moment as an IEP format. Feedback so far is fairly good.

Planning Templates for English and Maths

(1)
Weekly planning templates for English or Maths or both (adapted from something I received but tailored for my school). They are detailed but include space for: LI and SC, BLP Learning Muscles, AfL, Resources, vocab, evaluation, key questions and of course, the lesson. Also space at the end for individual records. Easily adaptable and can be used for individual lessons as well.

Learning Intention and Success Criteria Prog Model

Progression Model. I use these in every maths and English lesson. Once I've typed the required Learning Intention and Success Criteria into them, I copy and paste them into my IWB resource. I then print them out and put them into tents (piece of A4 card folded in half with part of a plastic wallet on the front to slip the paper into) and then place them onto the desks in front of the children. I then get them to refer to them as they are working so they can talk about what they need to do to improve their learning. Adapted from The Perfect Ofsted Lesson book by Jackie Beere.

The landscape document is used in my school for tracking interventions the teachers put into place for individual children. This can then be used at a LIFT meeting or as part of a statutory assessment as evidence as what has been put into place.<br /> <br /> The portrait document is a document I use for any intervention or discussion for SEN/vulnerable children. Again, this can be used as evidence.
