One page that tracks class participation, in class work, whether homework was turned in and interactions with others. The categories are rated from 1 to 5 and then totaled at the end of the week. It also has sections for both parent and student comments, plus space for a parent signature. There is a space at the top to write the class for which the chart is being used. 2. One page for summarizing daily behavior for all categories. There are 2 versions of the behavior chart, one in black and white and one shaded in light purple.
