First Day back 2021

A simple form time template for the first day back.

How to use the resource?
1.) Print as is – can be printed in black and white or colour
2.) Have students design their name and answer the
questions
3.) Fold the paper along the seam (arrow to arrow)
4.) Display how you like e.g.
a. Hang the name over the desk and the question
answers stuck flat to the desk so you can read them
b. Fold in half and have in a pyramid shape with name
facing you.
