Provision Map and Intervention Tracker

This is something I have been working on for my primary school. As SENCO, provision maps and intervention trackers are a useful tool to track interventions that are in place. Every school is different but I have made this so that all the provision maps and the intervention tracker are all on one document. It also automatically copies over data and evaluates it which saves a lot of time.
Our provision maps are completed every two weeks by the adults who leads the interventions. A baseline assessment is completed at the beginning of an intervention (mostly at the beginning of a term) and an assessment is completed at the end of the intervention (mostly at the end of a term).
I have made notes mostly on PT HT5 and Intervention Tracker tabs but have made notes on other tabs where things are different from PT.

Being a SENCO and full time class teacher I know the need for time saving strategies where possible so if this saves someone some time and can be tweaked to what you need for you school then I’m happy to share.

This did take me a few weeks on an off to code to what I needed it to be hence it being a paid resource.
