SEND Self-Evaluation Form

If you’re unsure about the quality of SEND provision currently provided, a good starting point is the SEND Self-Evaluation Form which will help you to reflect on and evaluate your current practice and plan strategies to better support pupils.

This SEND Self-Evaluation Form (SEF) resource is broken down into categories, including SEND Leadership, Quality of Teaching and Learning, Working with Parents, Assessment and Identification, Monitoring, Tracking and Evaluation, Efficient Use of Resources and Pupil Perspectives.

This fully editable word document resource is useful for a special educational needs and/or disabilities co-ordinator, as well as those line managing a SENDCo or for the senior leadership team to assess the quality of provision provided. This may also be a good tool to use with a SEND link governor or the wider governing body.
