<p>This flowchart is designed to show parents and staff the school’s process for identifying and supporting pupils with additional needs.</p> <p>The document follows the graduated response (Assess, Plan, Do, Review) and moves between four stages:<br /> Stage One: Quality First Teaching,<br /> Stage Two: SENCO Involvement<br /> Stage Three: SEND Register and Support Plan<br /> Stage Four: EHCP</p>

<p>A form for class teachers/TAs to complete when they have concerns over a pupil’s progress/needs. This is the first step when requesting SENCO input to ensure each step of identifying, monitoring and supporting needs are documented.</p>
